School shoes sponsor details

We are looking for families to shop with a designated family living in Woodlawn Homes in Painesville.  There is NO COST for your family or the Woodlawn Homes family, but the Woodlawn family will receive shoes for school.

Logistics:
WHEN: September 21, 2024 - Scheduled times between 10:30 AM - 12:30 PM
WHO: Family from Woodlawn Homes Painesville. Parent or
guardian and youths ages 3 – 18.
WHERE: Pickup at Woodlawn Homes Community Center next to 549 Sanders Ave, Painesville OH
SPONSORS: Couples or couples and children. A couple can be a pair of adults.

Details:

• Sponsor couple fills out the form above.
• Sponsor couple is notified by Catalyst Ministry when to meet Catalyst Ministry representative and Woodlawn family at Woodlawn Homes Community Center.
• A Waiver of Liability form will be signed at the community center (required before participation).
• Catalyst Ministry will provide the funds for the purchase of the shoes.
• Woodlawn family & sponsor couple go shopping.
• Woodlawn family returned to community center with new shoes for school.
• Sponsor couples must return any extra funds PLUS THE RECEIPT(s) to Catalyst representative.
